Vital Records

Bent County Public Health Vital Records issues birth and death certificates as mandated by Colorado
State law. Application may be downloaded below, or you may pick up in our oƯice.
How to Obtain Records: In person-same day service or by mail.
Mode of Payment: Cash, Check and Credit or Debit Card (2% convenience charge will be added for each
certificate).
Fees: Non-refundable. $25.00 for the first copy and $20.00 for each additional copy of the same
certificate if ordered at the same time. Expedite USPS Shipping is available for an additional charge.

Birth Certificates

Note: Due to a change in Colorado state regulation, beginning October 15, 2024, grandparents will no longer be eligible to order birth certificates for grandchildren based on relationships. Grandparents that provide a certified court order of guardianship or other legal/tangible interest documents are still eligible to obtain the birth certificate of their grandchild:
